Physician-led means accountable. It does not mean one person does every laser.

At R Med+Spa Durango, a board-certified physician is responsible for your plan, your settings, and your safety. Dr. Brian Rike, DO, performs Halo, BBL HERO Full 3-Step, Clear V, and Contour TRL. Andrea Rike, co-founder and specialist, performs Clear Silk, BBL BARE hair removal, BBL Forever Young photofacials, and Glacial Rx. If you want to know who will treat you, we tell you before you book.

Physician-led is not the same as physician-performed

Those two phrases get mixed together online, and patients pay for the confusion. Physician-performed means the doctor is the one using the device on that visit. Physician-led means the doctor owns the medical decisions even when a trained specialist is the one you see. R Med+Spa uses both, on purpose, and we will not hide the difference.

Dr. Rike performs the higher-energy and higher-judgment lasers himself: Halo hybrid resurfacing, BBL HERO Full 3-Step, Clear V vascular work, and Contour TRL. Those visits are not delegated. Andrea Rike performs Clear Silk, BBL BARE, Forever Young photofacials, and Glacial Rx. She is not a fill-in. She is the co-founder who built the patient experience and who treats those visits as her specialty.

Oversight still sits with the physician. Device choice, candidacy, and what happens if skin does not behave as expected are medical questions. Why judgment matters more than a device name

Sciton makes excellent platforms. The platform is not the plan. Two people can own the same handpiece and produce two different recoveries. Energy, pulse pattern, number of passes, cooling, and the moment you stop are where injuries or wasted visits actually live. That is judgment.

Halo is a hybrid fractional laser. It can be lighter or more aggressive. Stacked with BBL it can address pigment and texture together. Too little energy leaves you paying for a facial you could have skipped. Too much energy on the wrong skin, at the wrong time of year, is how people remember a laser for the wrong reason. Halo at this practice is physician-performed because those knobs are not decorative.

Clear V is a 1064nm vascular laser. It is aimed at a vessel, not at “redness” as a mood. Immediate darkening, bruising, or a skipped vessel are endpoints a physician should be watching, especially on the face. That is why Dr. Rike performs Clear V, not because the marketing sounds serious.

Contour TRL is tunable. Depth is a decision about your calendar and your skin, not a package upsell. BBL HERO Full 3-Step is a longer, physician-performed visit. Forever Young photofacials and Clear Silk are quieter, and Andrea performs them well because that is her work, not leftover work. The suite only helps you if someone is honest about which visit you actually need.

Durango sun, altitude, and dry air change the plan

Durango is not a coastal spa climate. Elevation means stronger UV. Outdoor time is part of why people live here. Dry air strips barrier function. Together, those things make pigment return faster and make aftercare less forgiving. A setting copied from a humid, low-elevation clinic is not automatically safe here.

That is why we talk about timing, hats, and sunscreen as medical aftercare, not as a product push. It is also why we will delay a laser if you are mid-tan, mid-trail-season, or not ready to protect treated skin. Physician-led care includes the unglamorous no. The no is part of keeping you from paying twice.

High country skin still wants laser. Sun spots, vessels, and texture show up earlier here than many patients expect. The answer is matching the device to the concern, then matching the calendar to Durango, not inventing a miracle recovery window.

A respectful note on other practice models

Plenty of good clinicians are not physicians. Nurses, physician assistants, and laser specialists can be excellent when they are trained, credentialed, and actually supervised. We are not interested in running down other people's licenses. Patients are not served by that.

What we object to is fog. A medical director who is never in the building is not the same as a physician who performs your Halo. A rotating tech pool is not the same as knowing Andrea by name. If another clinic can tell you, clearly, who treats you and who is on site if something is not routine, that clinic is doing right by you. Ask. Then decide.

Dr. Rike earned his Doctor of Osteopathic Medicine from the Arizona College of Osteopathic Medicine at Midwestern University. He has practiced medicine for more than a decade. Why does Halo or Clear V need a physician at the device?

Those treatments change collagen, pigment, or vessels in real time. Energy, endpoint, and when to stop are medical judgments. At this practice, Dr. Rike performs those himself rather than handing the handpiece to a rotating tech.

Is a nurse or laser-tech model unsafe?

Not automatically. Many clinics use trained specialists under a medical director, and that can be responsible when training, protocols, and real supervision are in place. What patients deserve is a straight answer about who is in the room. We give that answer in writing here.
